Jury Qualification Questionnaire - FAQ's

When are the Jury Qualification Questionnaires mailed?

Questionnaires going out in May, 2014 are for jury duty from October 2014 to September 2015.

Do I have fill out both sides of the questionnaire?

Yes, please remember to fill both sides of the questionnaire.

I need to write more than will fit in the box, can I attach a sheet of paper to the questionnaire with my answers?

Yes, you can attach another sheet to the questionnaire if needed.

If the person on the questionnaire is no longer living at the address on the questionnaire, what should I do?

Please write "No Longer At This Address" on the questionnaire and return it in the envelope provided. (If you know the persons new address please write that on the form and return it in the envelope provided.)

What if the person on the questionnaire is over 70 and does not wish to serve on jury duty?

Simply check the “No” box on question number 5.

What if the person listed on the questionnaire is deceased?

Please write that anywhere on the questionnaire and return in the envelope provided.

If I've been convicted of a felony do I have to put that on the questionnaire?

Yes, if you have a felony conviction you must list that on question #7 and list what State and what year the conviction took place.

I have a Medical or Religious reason for not serving on Jury Duty, where do I put that on the questionnaire?

If you have a Medical or Religious situation that prevents you from serving on Jury Duty please list those on question #4 on the front of the questionnaire and question #18 on the back of the questionnaire. You may attach another sheet if needed.

What If I don't live in Isabella County?

If you no longer live in Isabella County please list your new address on the application and return in the envelope provided.
